Case Management, Home and Community Based Services

The provision of specialized assistance based on the results of a comprehensive assessment for the individual desiring or needing help in selecting or obtaining services and in coordinating the delivery of the services.

Adult Day Care
Community based, non-residential service offered to individuals 18 years and over who are functionally impaired. Provide social and support services in a protective setting.

Adult Family Foster Care & Licensing
Provide foster care in a family type environment to developmentally disabled, physically handicapped, chronically mental ill, elderly individuals as an alternative to nursing home care. Available to Medical Assistance applicants and persons screened for skilled or intermediate care.

Basic Care Assessments
Case managers are responsible to assess whether an applicant/client meets the functional eligibility requirements for residence in a basic care facility.

Family Home Care
Payments available to relatives (spouse, parent or adult child) for the care of eligible elderly or disabled persons as an alternative to nursing home care.

Homemaker Services
The provision of non-personal care tasks such as housekeeping, laundry and shopping which enable the individual to maintain or develop the independence needed to remain in their home.

Personal Attendant Care
The maintenance, assistance, and/or supervision in any of the following health related care, self-care environmental and support safety care, and community support care on a 24 hour basis.

Respite Care
The provision of temporary relief to the individual's primary caregiver for a specified period of time. The caregiver is relieved of the stress and demands associated with continuous daily care.

Senior Adult Program
Case management, information, and referrals for the elderly and/or disabled.
